If you are receiving regular treatment, repeat prescriptions may be obtained using the right-hand side of your previous prescription form. Please tick each medication that you require.
These may be ordered
- In person at reception, allowing a minimum of 48 hours for collection
- By post, enclosing a stamped self addressed envelope, allowing 4 days for delivery
- By using the NHS app. Download here – NHS App – NHS (www.nhs.uk)
- By emailing in repeat prescription requests on: [email protected]
